To provide navigation and resource connection services, we collect information directly from you when you register and interact with the PHREE app:

• Contact Information: Full name, email address, telephone number, physical address, or mailing address.
• Essential Needs & Assessment Data: Information you share regarding your primary needs, which may include housing stability, food access, transportation, utility assistance, employment, financial assistance, and general health or wellness needs.
• Sensitive Information: Depending on the specific resources requested, you may voluntarily disclose sensitive personal information (such as disability status, family composition, or eligibility for public benefit programs).
• Communication and User Content: Notes, messages, attachments, and records exchanged between you and your assigned Navigator within the app.
• Identifiers & Technical Diagnostics: Unique user identifiers, device attributes, and basic diagnostic/crash data collected automatically to ensure app stability and performance.

2. How We Use Your Information
We collect and use your data strictly for App Functionality and core service delivery. We do not sell your personal information or use it for cross-app advertising or marketing. Your information is used to:

• Create, secure, and maintain your PHREE account.
• Pair you with a dedicated Navigator who can assist you with your goals.
• Evaluate and process referrals to third-party community-based organizations, public agencies, and essential service providers.
• Communicate with you via in-app notifications, SMS, email, or telephone regarding your tasks and referral progress.
• Diagnose technical issues, resolve bugs, and maintain system security.

3. How We Share Information (Referrals & Service Delivery)
PHREE operates on a collaborative care model. We share your information only under the following circumstances:

• Assigned Navigators: Your assigned Navigator has access to your profile, needs assessment, and communication history to provide guidance, manage tasks, and coordinate aid.
• Referral Partners & Service Providers: When you consent to a referral for a specific service (such as housing placement, food pantry access, or legal aid), we transmit necessary contact details and relevant intake data to that external service partner so they can deliver services.
• Service Follow-Through: Navigators may exchange status updates with referral providers (e.g., confirming enrollment or appointment attendance) to ensure follow-through.
• Infrastructure Vendors: Trusted technical service providers (such as secure cloud hosting, database management, and communications infrastructure) who are bound by strict confidentiality and security obligations.
• Legal Compliance: When required by applicable law, court order, or to protect the vital safety of an individual.

4. Third-Party Tracking and Advertising

• No Ad Tracking: PHREE does not track you across third-party websites and applications for targeted advertising.
• No Data Brokers: We never sell, rent, or trade your personal information to third-party data brokers or commercial marketing entities.

5. Data Security and Retention
We maintain administrative, technical, and physical safeguards designed to protect personal information against accidental, unlawful, or unauthorized destruction, loss, alteration, access, or disclosure. • Revoking Referral Consent: You may notify your Navigator at any time if you wish to withdraw consent for an active or pending third-party referral.
• Account Deletion: In accordance with Apple App Store guidelines, you can request the full deletion of your PHREE account and associated personal data directly from within the app settings or by contacting our privacy team.
